Admission Criteria

A patient and family are appropriate for Circle of Life Hospice when:

• Your physician and the hospice physician have certified a prognosis of six months or less if the disease runs its normal course
• You have agreed to stop seeking curative treatment
• You understand and have accepted the hospice philosophy of care
• You reside in Benton, Carroll, Madison, or Washington county (Residence is where you call home, including nursing homes and assisted living facilities)

Circle of Life provides care to eligible individuals regardless of age, race, religion, sexual orientation, diagnosis, financial status, or ability to pay.
