General Description: The Physician has responsibility for the medical aspects of the hospice program as delegated by the Executive Medical Director. Keeping in mind the mission of this organization, the Physician must be concerned with excellence in patient family care, serving as the primary liaison between the Hospice program and the medical community. Ensures the organization and its mission, programs, products and services are consistently presented in a strong, positive image to the medical community. Represents the organization to the medical community. Assumes a leadership role in the development of relationships with individual physicians, physician practice groups, hospitals, nursing homes, and other medical organizations, as appropriate. Provides the primary liaison between Hospice and appropriate national, state, and community-based medical organizations. Provides palliative care clinical services in the community. |

The Advanced Practice Nurse, working collaboratively with the Medical Director and other hospice and palliative care physicians, provides care to patients and families experiencing life-limiting progressive illness across the life span and in diverse settings. The APN will provide evidence-based physical, psychosocial, emotional and spiritual/existential palliative care, in collaboration with the hospice physician, to hospice and palliative care patients referred to Circle of Life. The APN may also conduct recertification visits, evaluating for and documenting relevant clinical information to be used by Medical Director or Hospice Physician as part of the hospice recertification process. Work may include weekend rotations. |
